Student Enrollment Information
Health Requirements
To comply with Kentucky
State Law, students entering McCracken County Public Schools for the first time
must have the following:
1_____ Physical
Examination
Exam must be completed on the
“Preventative Health Care Examination Form”
within one year prior to entry
into Kindergarten or Sixth Grade.
**Students entering a Kentucky school from out of state must have a
physical completed within 30 days of entry.
**Students entering a Kentucky school from a private school must
have a physical completed within 2 weeks of entry.
2_____ Current
Immunization Certificate*
This certificate must be
presented to the school in which the student plans to enroll. The certificate should have the following documented:
Kindergarten students:
DTaP 5 doses one
being AFTER the age of four
Polio 4 doses one
being AFTER the age of four
MMR 2 doses
Hib 4 doses
after 1 year of age; not required after five years of age.
Hep B 3 doses for students born AFTER
10/01/1992
Varicella 1 dose or
history of the Chicken Pox
Sixth Grade students or older entering Kentucky schools for the
first time:
Tetanus booster 1 dose at
6th grade (or higher) entry
*Certificates
from other states accepted
3_____ Certified
Birth Certificate
A copy of the birth certificate
must be presented for enrollment.
Copies of the hospital record of birth or wallet cards cannot be
accepted. An application is available
to those needing a certified copy.
4_____ Vision
Exam (Preschool and Kindergarten students ONLY)
Exam completed on the “Kentucky
Eye Examination Form for School Entry”
This must be submitted to the
school no later than January 1st of the first year that a three (3),
four (4), five (5), or six (6) year old child is enrolled in school.
5_____ Social
Security Card
A copy of the student’s social
security card must be presented for enrollment.
The forms can be obtained at
any McCracken County Public School.
Contact the school your child will attend and speak with the school
nurse for information.
